Please see the message below from Galadent who will be at Troy Park for mouthguard fittings:
Wednesday 1st July 2020 4.00pm – 6.00pm
Firstly, please go online to
www.galadent.com.au to make your child/children’s appointment. Click on ‘Mouthguard Booking’ and follow the prompts accordingly. If you are booking in
siblings, you are required to make an appointment for each child. Once you have successfully booked an appointment, a confirmation email will be sent to you.
Due to the COVID-19 restrictions still in place, to keep appointments running on time to avoid queues, it would be appreciated that while you are on Galadent’s website you also complete their online ‘Mouthguard Form’. Not having to complete a hardcopy form on the night, will enable the process to move along steadily. To do this, please click on ‘Mouthguard Form’ then on ‘Download Form’ and save it before you complete your child’s details (some browsers will not save your typed information unless you have done this first). If you have booked in siblings, you will be required to complete an online form for each child. Please note that completing the online form, does not mean that Galadent automatically receives this information. This only enables you to print your typed information instead of filling it in yourself.
Please ensure that you have printed both pages of the online form to bring with you to your child/children’s appointment. If you are unable to print the form, you can email it as an attachment to,
info@galadent.com.au and they will print it for you. We ask that you only do this if you are unable to print it at home. There will be hardcopy mouthguard forms available on the night if you are unable to complete an online form. If your only option is to complete a hardcopy form, please arrive a few minutes earlier to allow time.
All the types, colours and costs of the various mouthguards are outlined in the online ‘Mouthguard Form’. Payment is required on the night, so it would be appreciated if you could come prepared to avoid delays. You can either fill in your credit card details in the online
mouthguard form, have the required cash amount or have a completed cheque (please make cheque payable to Galadent Dental). If you are unsure of the cost, due to not being confident with the type of mouthguard you should select, then Galadent will go through the cost with you on the night.
The mouthguards will be delivered back to the club with their receipts, which is a 2 week turnaround. Please ensure to claim your mouthguard rebate through your private health ancillary cover. For the parents who are members of HBF, Galadent are a Members Plus provider, so this will entitle you to a higher rebate.
We appreciate that this is short notice for you to get organised, but we do ask that you go online to book in your appointment/s, as soon as possible.
