From Galadent:

Wednesday 31 March, 3.30-6.30pm @ Troy Park.

After a delayed start to our season last year, we are now back into our normal routine, so it’s time for mouthguards. We appreciate that parents may find that their child’s mouthguard still fits but please be mindful that it is not a full year since we did mouthguards last year. Galadent has asked you to consider that until your child has all their adult teeth, their mouths are always changing. For this reason, there is a high possibility that they will outgrow their mouthguard from last year, midway through the football season. Therefore, it is recommended that you replace their mouthguard now to avoid disappointment.
If you are wanting a mouthguard for your child, please go online to www.galadent.com.au to book in their appointment. You are welcome to bring along siblings, but you will be required to make an appointment for each child. Click on the ‘Mouthguard Booking’ link and follow the prompts accordingly. Once you have successfully booked an appointment, a confirmation email will be sent to you.
While you are on Galadent’s website you are also required to complete their online ‘Mouthguard Form’. Please ensure you SAVE the form before you complete any details (some browsers will not save your typed information unless you have done this first). If you have booked in siblings, you will be required to complete an online form for each child. You will need to PRINT both pages of the form to bring with you to your child/children’s appointment. If necessary, there will be hardcopy mouthguard forms available on the night, that can be completed just prior to your child/children’s appointment.
All the types, colours and costs of the various mouthguards are outlined in the online ‘Mouthguard Form’. There is a brief explanation for each type of mouthguard, to assist you in selecting the correct type. If you are still unsure which type, this can be determined when your child/children are having their impression done.
Payment is required on the night by either filling in your credit card details on the mouthguard form, paying in cash or a cheque (made payable to Galadent Dental). An itemised account/receipt will be emailed directly to you, so please ensure that you have completed your email address on the mouthguard form.
The mouthguards will be delivered back to the club 2 weeks from impression night. This falls in the 2nd week of the school holidays. Be sure to follow up at the club, when training resumes, for your mouthguard.
